At the Reception Desk you will be requested to complete the "Admission Form" in order to provide your personal details.


More specifically, you will be requested to register and submit the following information:

  • Patient’s Name/ Surname (along with ID or passport)

  • Name of Companion (along with ID or passport)

  • Address of residence and phone number (mobile/landline) of the patient and the companion.

  • Selection of Room type (Suite, Single Bed or Double Bed)

  • Declaration of insurance Medical Card / member of a Health Fund / out of pocket payment.

  • Physician’s Referral form and relevant doctor instructions regarding your admission.

  • Deposit Payment depending on the patient cause of admission


Afterwards, the Patient Relation Services staff will assist and ensure your transportation to the assigned Patient Room, where the nursing staff will welcome you, and assist you in making yourself confortable and take over your medical care needs.

Every patient is kindly advised to pay attention to the instructions and exhortations of the nursing staff. The patient is also advised to cooperate and respond to the caring staff with patience, having in mind that the nursing staff will be asking various questions regarding the patient's health, seeking for answers.

The admission process is of the greatest importance, ensuring a patient's smooth, comfortable and safe stay at the hospital.
